Appointments

Call our office to book your appointment, or request an appointment online. In the event we cannot honor your appointment request, our staff will contact you.

Are You a new Patient?

To ensure your first visit runs smoothly, we recommend you do the following:

• Sign up on the patient portal and update your personal and medical information or complete and bring the New Patient form with you to your appointment (if applicable, please also complete the Consent for Treatment of a Minor). 
• Either send to our office or bring to your appointment copies of current medical records, including recent ER visits or hospitalizations.

Are you scheduling a physical?

A physical is a preventative health visit. This is a time for you to discuss your entire medical history with your doctor, including immunizations and family history. If you have a new health issue that needs to be addressed (e.g., back and joint pain, high blood pressure, depression or anxiety, etc.) please schedule a separate appointment. Doing so enables your physician to devote adequate time to your preventative health needs.

Prescription Refills

Routine refills are performed during normal weekday office hours.
